• IRS Letter indicating when EIN was issued

Sole Proprietors please send one of the following:

• Stamped and Filed copy of your Schedule C
• IRS Letter under Sole Proprietorship's name showing the DBA, Trading or Fictitious Name
• Business License showing Sole Proprietor and DBA Name (must be active)

Lastly, we ask that you provide a current phone bill or commercial lease which shows your complete physical address and phone number.

Below are some examples:

• Current phone bill listing your company name, phone number and physical address
• If your utility bill does not list your physical address please include the Lessee/Lessor and Signature page of your Commercial Lease
